In this procedure, the scarred or diseased portion of the urethra is removed, and the remaining portions of the urethra are joined together by fine sutures. This treatment is for short strictures that have bulbar urethra (a segment of the urethra present inside the scrotum and end up to the prostate), and a part of the scarred urethra is removed completely. Then the two cut ends of the urethra are joined together. Removal of a short segment of the urethra and joining of the ends together gives excellent results that show the best durable results. End to end urethroplasty cannot be performed for the penile urethra or long strictures of the bulbar urethra due to the high tension produced between two ends during their joining. This will lead to surgical failure as well as curvature and shortening of penile.
In graft urethroplasty, the size of the narrow urethra is enlarged by patching grafts rather than a complete replacement. In this procedure, mucosal tissue from cheeks (lining of the mouth) of the patient is used for the formation of the new urethra. A graft from the mouth termed as a buccal graft that is a wet and pliable tissue is used as an excellent substitute for the urethra. Management of most bulbar urethral strictures is done by buccal repair due to its ease of technical performance, high success rate, and reliability. Pain is minor in this procedure, and the results are excellent.
